Contractors Application

Below are the requirements needed to apply. Once you have all your documents ready, you can Apply Below. 

Application Requirements

  • Copy of your occupational license.

​

  • Copy of you certificate of liability insurance.   We need to be listed as the certificate holder "RTTB 3914 N. 301 HWY., Suite 700 Tampa, FL. 33619"

  • A copy of your workers compensation. If you qualify for an exemption, provide that.

​

  • Complete the ACH Form for providing payment information if needed.

​

  • Complete and Sign the Sub-Contractors’ Agreement

​

  • Complete and sign the Sub-Contractors Photo & Invoice Agreement
